a year in the life of our social prescribing team

Front cover of report, 'A year in the life of the social prescribing team'

Our team of link workers and counsellors is the largest in the North East region, with social prescribers in every GP practice in Newcastle, and includes a dedicated refugee and asylum seeker social prescribing team and qualified counsellors.

Across 2025, our team dealt with referrals from more than 5000 patients, and continue to support people across the city.

You can read some of their stories in our report, ‘A year in the life of our social prescribing team‘.

Refugee and asylum seeker service evaluation 2025

We have carried out a full-service evaluation of the Refugee and Asylum Seeker (RAS) Social Prescribing Link Worker (SPLW) service in Newcastle. Our RAS SPLWs have worked across both hotel and dispersed accommodation settings to deliver social support and signposting to service users, connecting them to activities, and helping to navigate the health and social care system.

This report aims to reflect on outcomes monitoring under the Health Inequalities grant, in addition to service review and evaluation.
